Do Objective Diagnostic Criteria Matter in Psychiatry?

One of the criticisms antipsychiatry folks like to make of psychiatry is its lack of objective diagnostic criteria. In other words, there’s no blood test that says you have bipolar disorder or schizophrenia.

And this is true. While today we do have blood tests for biological markers indicative of mental illness diagnosis, there is no hard and fast test that can diagnose a psychiatric disorder (except Huntington’s, for which we have discovered a gene).

The fact of the matter is no matter what is written in the Diagnostic and Statistical Manual of Mental Disorders (DSM) or what blood we draw or which scans we do nothing diagnoses a person properly except a trained psychiatric professional.

But that doesn’t mean there’s nothing objective or meaningful about it.

Why Write About Doctor-Patient Communication?

I’m writing this book because of the plethora of mental health questions I get on the subject. I believe people with bipolar disorder, depression and other mental illnesses don’t get optimal care due to dysfunctional medical relationships. This is not necessarily the fault of the doctor / psychiatrist or the patient. It just means the relationship isn’t as good as it could be. Think of it like couples counseling between doctor and patient.

Mental Illness Articles You Should Read

As per the usual, however, I plow through my own research materials like a crazy person possessed. So I do know of many excellent articles you should be reading.

Check out these articles from Breaking Bipolar and other great sources:

Schizophrenia Awareness Fights Fear and Stigma – This is a video embedded in a post at Breaking Bipolar and has been quite well-received. Please take a look at this short piece and learn something about a mental illness that most people irrationally fear.
Do Mental Illness Diagnoses Matter? – Sometimes people argue that diagnoses are too broad and overlap too much. I’ve even heard of doctors not wanting to tell their patients their diagnosis because they don’t want them “hung up on it.” But I argue that even if ill-fitting mental illness diagnoses do matter. (Also read a good piece about addiction terminology. Did you know “addiction” isn’t in the DSM?)
Doctors Asking Questions About Suicide – Sometimes it seems like the questions doctors ask about suicidal thought are useless, but I don’t think they are.
How to Handle the Common Symptoms of Schizophrenia – This is an amazing document by the British Columbia Schizophrenia Society. This is so clear and so useful that I think it’s a must-read for anyone that has or knows someone with any mental illness. It will not just tell you about schizophrenia and psychosis, but will tell you about mental illness thought processes in general. (Also, How Friends and Families can Handle Psychosis.)
Scientific and Consumer Models of Recovery in Schizophrenia: Concordance, Contrasts, and Implications – Interesting paper that discusses various treatments including medication and non-medication based. Yes, it’s long but there are some interesting bit in there.

Newly-Diagnosed Bipolar – What to Do

One of the things that happens because I write about bipolar disorder so much, is that people ask me a lot of questions about what they should do with their bipolar treatment. I understand this. People feel like they don’t know what to do. And often people just don’t have a good relationship with their doctor/psychiatrist.

No One Can Make Mental Illness Treatment Decisions But You

The thing is, I can’t tell you what to do with your mental illness treatment. I can give you information, I can try to help, I can suggest reliable, mental health resources, but you have to make the treatment decision. It’s a scary thing I know. I still hate it myself.

And the people who have it the toughest are the ones who have just been diagnosed with bipolar disorder or another mental illness. So here’s what I suggest you do if you’ve just been diagnosed with bipolar disorder.

Psychiatric Disorders in Children – Diagnosed and Medicated

I have bipolar-disorder-type-II-ultradian-cycling. I diagnosed myself when I was 20 years old, and once I finally agreed to see a doctor, he agreed sometime thereafter. My diagnosis was fairly easy for me. I’m very self-aware and I could pick out discrete moods and swings. But as a 20-year-old, in university, using research, and having a fairly high IQ, this is not terribly surprising. If I were five-years-old, the picture would have been a little different.

Epidemic of Children Diagnosed with Mental Illness

There is an epidemic of children, as young as two, being diagnosed with psychiatric disorders in North American right now. It’s made the cover of Time magazine and countless articles have been written on the phenomenon.

So, Antipsychotics are Now Approved for Children

It was once thought that disorders like bipolar did not occur before adulthood, but thoughts on this seem to be changing as diagnoses go up and more drugs are approved for treatment of children.

Antipsychotics FDA-approved for use in children (under 18) is:

Quetiapine (Seroquel) – schizophrenia, ages 13-17; bipolar I, ages 10-17
Olanzapine (Zyprexa) – schizophrenia and bipolar I, ages 14-17
Risperidone (Risperdal) – schizophrenia, ages 13-17; bipolar mania, ages 10-17; autism, ages 5-16

And so on. And of course, doctors are free to prescribe any medication off label to children just like adults.
